Eligibility

In order to receive immunizations for your child (0-18 years of age) at the health department, your child must fall within one of the following groups:

  • Uninsured (no health insurance coverage) Medicaid (Hoosier Healthwise)
  • Limited health insurance (underinsured): Child has health insurance coverage, but the policy does not pay for vaccines/immunizations.*
  • American Indian/Alaskan Native (regardless of insurance coverage)

*NOTE:  It is the responsibility of the parent/guardian, NOT the health department, to check with the child’s insurance company to determine whether or not vaccines/immunizations are covered under their policy. Please do so prior to making an appointment. Also, insurance deductables are not taken into account when determining a child’s eligibility. For example, if the child’s insurance does pay for vaccines but there is a high deductible on the policy, the child is still not eligible to receive vaccines at the health department.

All vaccines at the health department are provided by the federal Vaccines for Children Program (VFC) and are provided by the Indiana State Department of Health (ISDH). Therefore, all rules and regulations regarding vaccine eligibility are also set forth by the ISDH, not the local health department.

ADULT IMMUNIZATIONS: Certain adult immunizations may be available at the health department, but on an extremely limited basis as determined by criteria set forth by the ISDH.  Please call 219.285.2052 with any inquiries regarding adult immunizations.
